I, hereby grant permission for my child to participate in the Bergen Biddy Basketball Clinic, League, or Camp. In consideration for allowing my child to participate in the Program, I, on behalf of myself and my child, hereby agree to the following terms and conditions:

Assumption of Risks: I understand that participation in basketball activities involves inherent risks, including but not limited to the risk of injury. I acknowledge that my child may be exposed to physical contact, falls, and other potential risks associated with the sport of basketball.

Medical Treatment Authorization: In the event of an injury or medical emergency, I authorize the staff, coaches, or medical personnel associated with the Program to obtain necessary medical treatment for my child. I also understand that I will be responsible for any medical expenses incurred.

Release of Liability: I release and discharge Bergen Biddy, its officers, directors, employees, volunteers, and any sponsors from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in the Program.

Photography and Publicity Release: I grant permission for Bergen Biddy to use photographs, video recordings, or other likenesses of my child for promotional and publicity purposes related to the Program.

Code of Conduct: I understand that my child is expected to adhere to a code of conduct during their participation in the Program. Any violation of the code of conduct may result in my child's removal from the Program.
I have read and understood the terms of this waiver, and I voluntarily agree to its terms.
